Yes — hospital discharge is one of the highest-risk transitions in aging. We work with the hospital case manager, review discharge instructions, evaluate the home setup, coordinate any home health or equipment, and make sure the family understands what to watch for.
For active Portland Westside crisis cases, same-day phone support and next-day in-home is the standard. Existing clients always get priority.
